We work with individuals, parishes and schools to help young people develop an enduring relationship with Jesus and discover their place in the Church and in the modern world. Please pray for us as we strive to evangelize, educate, inspire, encourage and support all the youth and young adults of the Diocese.

Manuals & Forms

Catholic Mutual Group, the company which insures the Diocese of Fargo and all our parishes, has published two manuals to assist you in the details of any youth event or trip which will take you away from your parish. All chaperones and other volunteers who work with youth must follow the policies given in the Diocese of Fargo’s Code of Conduct.

Use this parental consent and medical waiver form for all events which your youth will attend away from your parish. It is based on the generic form supplied by Catholic Mutual and has been modified for the Diocese of Fargo. A parish simplifies needs to fill-in their name and a description of the event. (The document was written in Microsoft’s Word 2003 format. Please contact the Youth & Young Adult Office if you need this document in a different format.)
